The Community Health Taskforce brings together physicians, nurse practitioners, nurses, behavioral-health professionals, care navigators, community health workers, rehabilitation professionals, hospital-based specialists, home-care teams, hospice and palliative-care professionals, pharmacy resources, diagnostic services, social-service professionals, and community organizations.

Our purpose is to help individuals and families move through healthcare more efficiently by identifying barriers, organizing needs, connecting clients with appropriate professionals, and supporting follow-through across medical, behavioral-health, home, and community settings.

Members of the Taskforce may work directly within Comprehensive Healing Solutions or may participate through independent healthcare, hospital, community, home-care, specialty, diagnostic, rehabilitation, or support organizations.

Being part of the Community Health Taskforce does not mean every professional is employed by Comprehensive Healing Solutions. Each professional or organization remains responsible for services performed within its own license, scope of practice, credentialing, facility requirements, and payer rules.

How Referrals Work

Some services can be accessed directly. Other services require a physician or other qualified practitioner order, referral, certification, prior authorization, or documented medical necessity.

For Medicare beneficiaries, referral requirements vary considerably. Under Original Medicare, a referral to see a specialist is generally not required in most cases, while Medicare Advantage plans may require referrals depending on the plan type and network. Certain services and supplies—including some DMEPOS, laboratory, imaging, and home-health services—have specific ordering or certification requirements.

When clinically appropriate and permitted by Medicare, the patient’s insurance plan, state law, and the receiving provider, a Comprehensive Healing Solutions physician or nurse practitioner may evaluate the patient and issue an appropriate referral, order, or recommendation. CMS recognizes eligible physicians and certain non-physician practitioners, including nurse practitioners, as Medicare ordering and certifying professionals for specified services.

A referral or order does not guarantee acceptance, coverage, authorization, or payment.

Medical & Primary Care Team

1. Family Medicine Physician

Provides comprehensive medical evaluation across the lifespan, manages common acute and chronic conditions, coordinates specialty care, reviews medications, orders diagnostic testing, and makes referrals when medically appropriate.

2. Internal Medicine Physician

Manages adult medical conditions, particularly patients with multiple chronic illnesses, complex medications, and conditions requiring coordination among several specialists.

3. Geriatric Medicine Physician

Focuses on older adults with frailty, multiple chronic conditions, falls, cognitive changes, polypharmacy, functional decline, and complex care needs.

4. Nurse Practitioner

Evaluates patients, manages chronic and acute conditions within scope, orders appropriate testing, prescribes when authorized, coordinates care, and may issue referrals or qualifying orders when permitted.

5. Physician Assistant

Provides medical evaluation and treatment within authorized scope and practice arrangements, assists with chronic-condition management, diagnostic testing, and specialty referrals.

6. Registered Nurse

Supports clinical assessment, patient education, medication-related follow-up, care-plan implementation, transition support, symptom escalation, and communication with the medical team.

7. Licensed Practical/Vocational Nurse

Supports nursing care, medication-related tasks within scope, patient monitoring, documentation, and communication of changes to supervising clinicians.

8. Medical Assistant

Supports medical intake, documentation, appointment preparation, routine measurements, administrative follow-up, and communication between patients and clinicians within assigned duties.

9. Clinical Pharmacist

Reviews medication regimens, potential interactions, adherence barriers, medication-access issues, and supports prescribers with medication-management recommendations.

10. Consultant Pharmacist

Provides medication-regimen review for patients with complex medication use, long-term-care needs, or medication-safety concerns.

Evaluates heart disease, coronary artery disease, heart failure, hypertension complications, rhythm problems, chest symptoms, and cardiovascular risk.

Specializes in abnormal heart rhythms, pacemakers, implantable cardiac devices, atrial fibrillation, and complex rhythm disorders.

Performs specialized procedures for coronary and vascular disease, including catheter-based evaluation and intervention when indicated.

Provides surgical evaluation and treatment for conditions involving the brain, spine, spinal cord, and peripheral nerves.

Focuses on stroke prevention, acute and post-stroke care, cerebrovascular disease, and secondary risk reduction.

Treats Parkinson disease, tremor, dystonia, and other disorders affecting movement and coordination.

Evaluates recurrent seizures, epilepsy, seizure medications, and complex neurologic events.

Provides education about Medicare coverage pathways, plan rules, and benefit questions within applicable licensing boundaries.

Assists with understanding eligibility pathways and connecting individuals to appropriate state or community benefit programs.

Helps veterans identify VA, community-care, benefits, transportation, caregiver, housing, and other veteran-related resources.

Provides qualifying equipment such as hospital beds, wheelchairs, walkers, oxygen systems, and other medically necessary devices when applicable requirements are met.

Some DMEPOS items require a treating-practitioner order, and certain items also require a qualifying face-to-face encounter and written order before delivery.

Provides oxygen systems and related equipment when medical-necessity and ordering requirements are met.

Supports access to wheelchairs, walkers, scooters, and other mobility equipment based on clinical and payer requirements.

Prepares and supplies qualifying infusion medications and related equipment for home administration.

Monitors patients receiving certain blood-thinning therapies and supports safe dosing and follow-up.

124. Diabetes Education

Provides structured education on diabetes self-management, monitoring, medication use, nutrition, and complication prevention.

125. Ostomy Care

Supports patients with colostomy, ileostomy, or urostomy needs, including supplies and education.

126. Continence Care

Evaluates and supports bladder and bowel continence-related needs.

127. Respiratory Therapy

Supports breathing treatments, oxygen therapy, ventilatory equipment, airway-management education, and respiratory-device use.

128. Dialysis Services

Provides renal replacement therapy for patients with advanced kidney failure.

Supports qualifying patients performing dialysis in the home setting.

134. Utility Assistance

Connects patients with programs addressing electricity, water, heating, cooling, and other utility needs.

135. Caregiver Support

Provides education, resource connection, respite pathways, and practical support for caregivers.

Connects older adults with socialization, meals, education, activities, and local support programs.

137. Disability Resource Navigation

Connects individuals with disability-related services, benefits, equipment, advocacy, and community supports.

138. Legal Resource Referral

Connects clients with qualified legal-assistance organizations when legal needs are identified. The Taskforce does not provide legal representation unless a properly licensed professional independently does so.

139. Financial Resource Navigation

Helps identify assistance programs and financial-support resources without providing individualized financial advice.

140. Scam and Exploitation Support

Helps identify potential financial exploitation or scams and connects patients to appropriate protective, law-enforcement, legal, or social-service resources.

141. Adult Protective Services Coordination

Supports appropriate reporting and communication when abuse, neglect, self-neglect, or exploitation concerns meet applicable reporting standards.

142. Family Caregiver Education

Helps families understand caregiving responsibilities, available resources, safety considerations, and support pathways.

143. Grief and Bereavement Support

Connects patients and families with grief counseling, bereavement programs, spiritual care, and community resources.

144. Spiritual Care / Chaplaincy

Provides spiritual and emotional support consistent with the patient’s preferences and beliefs.

145. Community Outreach

Helps individuals access educational events, screenings, community programs, and healthcare resources.

How the Taskforce Works

The Community Health Taskforce uses a coordinated process:

1. Identify the need
A client, caregiver, PCA, healthcare worker, hospital team member, community health worker, navigator, or clinician identifies a concern.

2. Review the concern
Comprehensive Healing Solutions reviews the information and determines which type of professional or resource may be appropriate.

3. Clinical evaluation when required
When the service requires medical evaluation, medical necessity, an order, certification, or referral, an appropriate physician, nurse practitioner, or other qualified clinician evaluates the patient.

4. Referral or order when appropriate
If the patient qualifies and the clinician determines the service is appropriate, the clinician may issue the necessary referral or order within their lawful scope and Medicare/payer requirements.

5. Connect with the appropriate Taskforce discipline
The individual may be connected with a medical specialist, behavioral-health professional, rehabilitation provider, hospital department, home-care service, diagnostic provider, community resource, or another appropriate Taskforce participant.

6. Follow through
Our care-navigation and support teams can help identify barriers, prepare for appointments, track unresolved issues, and support communication among the patient, caregivers, and appropriate professionals.

Depending on the professional, payer, and service, patients may be able to directly access certain:

• insurance-information resources;
• food or transportation resources;
• certain outpatient professionals.

Some Services May Require an Order or Referral

Examples may include:

• home health;
• certain diagnostic imaging;
• laboratory services;
• DMEPOS;
• physical, occupational, or speech therapy depending on applicable requirements;
• specialist services under certain Medicare Advantage plans;
• hospice;
• certain infusion services;
• certain rehabilitation programs;
• other services requiring medical necessity, certification, prior authorization, or practitioner orders.

CMS specifically recognizes ordering/certifying requirements for DMEPOS, clinical laboratories, imaging, and home health, among other services.

A Comprehensive Healing Solutions physician or nurse practitioner may evaluate a patient and, when clinically appropriate and permitted by applicable law and payer requirements, issue referrals, orders, or certifications for qualifying services. Medicare coverage is ultimately determined by applicable federal and state law, Medicare coverage policy, local coverage rules, and the beneficiary's specific plan.
